Case Management
If you are living with HIV/AIDS let us help…
Volunteers of America’s HIV/AIDS Services program brings hope to individuals living with HIV/AIDS. Through case management programs such as Ryan White, individuals living in East Baton Rouge, West Baton Rouge, East Feliciana, West Feliciana, Iberville, Pointe Coupee, Ascension, Livingston and St. Helena Parishes receive care coordination, medication, housing, dental and transportation assistance. Case Managers can also provide assistance with applying for Social Security, Medicaid, Medicare, patient assistance programs, food stamps, and unemployment benefits. Peer support, mental health and substance abuse services are offered as well as a support group just for men living with HIV/AIDS.
The HIV Medicaid Case Management program provides services to eligible individuals. Activities include intake, assessment, comprehensive plan of care, building/implementing supports, monitoring support strategies and case management transition/closure.
