Paving The Way To Success

Volunteers of America has provided support to people with disabilities since the 1970s.  Support Coordination has existed since 1992.  The goal of the program is to provide opportunities for individuals and their families to accomplish their goals.  Individuals are able to receive a full range of services which include medical, social, educational, and other community resources.  The individual, along with a coordinator develop a "road map" to help accomplish set goals. Daily Life

The targeted population to receive Support Coordination services are individuals 65 years of age and older.  They must meet Medicaid financial eligibility and the criteria for admission to a nursing facility. This program is also offered for people who are age 22-64 who are disabled according to Medicaid standards or SSI criteria.

Support Coordination program helps achieve desired outcomes by helping consumers discover, prioritize and achieve desired personal outcomes by helping them:

  • Choose their own personal goals
  • Choose their daily routine
  • Choose where they live
  • Choose services
  • Exercise their rights
  • Ensure confidentiality
